Submittal Qualifiers: Does candidate have current or prior discipline on their professional license or certification? If yes, please provide explanation.Has the candidate ever been convicted of a crime other than a minor traffic violation?Does candidate have an LPN license in the State of South Dakota or Compact State?Does the candidate have a minimum of 2 years of experience working in a Long Term Care setting?Does the candidate have an active BLS issued through American Heart Association? Requirements: Resume Must include candidate's education (Including month/year of graduation), & complete work history from the last seven (7) years. All work history should include facility worked at, not the agency name.Skills Checklist Completed within one (1) year of submittal for specialty where candidate is submittedTwo (2) References Two (2) references from work history occurring within the past three (3) years. At least (1) one reference must be from a supervisor.Online verification of professional license or certification Verified within 30 days of submission. Must be signed by MD, DO, DC, NP, or PA.MMR Shown by proof of two (2) MMR vaccinations or positive IGG titers.Varicella Shown by proof of two (2) vaccinations, positive IGG titer, or provider verified history.Hepatitis B Shown by proof of vaccination series, positive surface antibody titer, or declination.Influenza For Assignments occurring between 10/1-3/31. Declination form is permitted.Tdap Administered within ten (10) years prior to start date.COVID-19 Shown by proof of vaccination(s) and any booster(s) that have been administered. Charting is Point Click Care and utilize walkie talkies for communication.  They utilize med carts (bubble packs) for medication distribution.  Day staffing:  (1) RN, (1) Med Aide, (2) CNAs and (1) Bath Aide; Night staffing: (1) RN or LPN, (2) CNA until 10:00PM.  After 10:00PM, (1) RN and (1) CNA. Misc Notes: Minimum of 2 years experience working as an LPN in a Long Term Care setting is required.  Will float to Assisted Living as needed for medication pass.  Requirements:  Licensed as an LPN in the State of South Dakota or compact state and BLS issued through American Heart Association are required.  Shift:  11.5-hour shifts;  6:00AM-6:00PM or 6:00PM-6:00AM, will work every other weekend.  Schedule is provided one month in advance.  Dress code:  Professional scrub top and bottom must be worn at all times but may be any color.  No other facility logos or names may be displayed on scrubs.  Facility provided badge must be worn at chest level, not at the waist.  Nails should be an appropriate length for resident care.  Acrylic nails are not permitted.
