**Job Title: Home Health Physical Therapist**
**Location:** Walnut Creek, Concord, Rodeo, Pinol, El Sobrante, CA
**Job Description:**
We are seeking a dedicated Home Health Physical Therapist to join our team, focusing on geriatric care. This 13•week contract, with potential for extension, involves a 40•hour workweek, Monday to Friday, typically from 8:00 am to 5:30 pm.
**Key Responsibilities:**
• Provide quality physical therapy to home health patients.
• Manage a caseload of about 6 patients daily, with scheduling support.
• Collaborate with RNs and OT/PT staff for holistic care.
• Use EMR systems like Oasis and HCHB; training available.
**Requirements:**
• Valid CA Physical Therapist license.
• Basic Life Support (BLS) certification.
• At least 1 year of home health experience; geriatric experience preferred.
**Additional Information:**
• No on•call duties.
• Potential for overtime and contract extension.
• Flexible scrub color; PPE details to be determined.
