**Job Title: Home Health Physical Therapist • Days**
**Location:** Brenham, TX
**Job Description:**
Join our client's team as a Home Health Physical Therapist, providing essential care to patients in their homes. This role requires a dedicated professional with a strong background in home health and expertise in HCHB/OASIS documentation.
**Key Responsibilities:**
• Deliver high•quality physical therapy services to home health patients.
• Complete all documentation on the same day, using Homecare Homebase (HCHB).
• Manage a caseload of approximately 20•25 cases per week, achieving productivity points of 26•28 on average.
• Attend mandatory in•person office meetings once a week.
• Maintain flexibility to accommodate patient needs, including potential early or late visits.
**Requirements:**
• Valid PT License and BLS Certification.
• Minimum of 2 years of experience in home health.
• Proficiency in HCHB/OASIS, with experience in Oasis E after January 2023.
• Ability to work a 13•week contract, 40 hours per week, with no on•call or overtime requirements.
• Must live within 30 minutes of the territory or secure nearby housing if traveling.
**Additional Information:**
• Orientation includes 2•3 days of training and team introduction.
• Scrub color is flexible.
• All requested time off must be pre•approved and made up.
