**Job Title: Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) • Home Health Services (Day Shift)**
We are currently seeking a dedicated and highly skilled Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) to join our esteemed home health care team in Katy, Texas. The successful candidate will be required to reside within a 30•minute commute of their designated service area to ensure timely and efficient patient care delivery.
**Key Responsibilities:**
• Provide comprehensive home health care services, ensuring meticulous and timely documentation of all patient interactions and care activities on the same day.
• Manage a caseload of 20•25 patients per week, consistently achieving a productivity benchmark of 26•28 points.
• Utilize Homecare Homebase (HCHB) and the Outcome and Assessment Information Set (OASIS) for effective patient management and care coordination.
• Participate in weekly in•person meetings at the office to discuss patient care strategies and team objectives.
• Demonstrate flexibility in floating to different areas as needed and fulfill weekend service requirements as part of the role.
**Essential Qualifications:**
• Possession of a valid LPN license and current Basic Life Support (BLS) certification.
• A minimum of two years of experience in the home health care sector, with a proven track record of delivering high•quality patient care.
• Proficiency in using HCHB and OASIS, with specific experience in Oasis E documentation post•January 2023.
• Strong skills in managing a diverse caseload effectively and efficiently.
**Contractual Details:**
• This position is offered as a 13•week contract, with a standard 40•hour workweek.
• There are no on•call duties or overtime requirements associated with this role.
• All requests for time off must be pre•approved to ensure continuity of patient care.
**Additional Information:**
• The role includes a comprehensive orientation program spanning 2•3 days to ensure a smooth transition into the team.
• Flexibility in scrub color is permitted, and all necessary personal protective equipment (PPE) details will be provided.
• The branch operates from 8 am to 5 pm, with the possibility of early or late patient visits as required to meet patient needs.
