Job Title: Home Health Licensed Vocational Nurse (LVN) • Day Shift
We're seeking a flexible, experienced LVN for a 13•week home health care contract. This role demands excellent time management and a dedication to quality patient care.
Responsibilities:
• Full•time work, with potential weekend shifts.
• Manage 20•25 home health cases weekly.
• Complete same•day service documentation.
• Attend weekly in•person meetings.
Requirements:
• Active LVN License.
• 2+ years of home health care experience.
• Proficiency in OASIS documentation and Homecare Homebase.
• BLS Certification.
• Continuous employment, with no more than a 1•year gap.
Preferred:
• Experience with Oasis E.
• Ability to manage 26•28 productivity points.
Additional Info:
• Mileage reimbursement.
• Flexible scrub color.
• 2•3 days of orientation.
• Office hours: 8am • 5pm, patient visits may occur outside these hours.
