**Job Title: Home Health • Speech Language Therapist**
**Location:** San Diego, CA
**Job Description:**
We are seeking a Speech Language Therapist for home health services. Deliver quality speech therapy to patients in their homes, focusing on communication and swallowing improvements.
**Responsibilities:**
• Assess and create personalized treatment plans.
• Conduct therapy sessions for communication and swallowing.
• Collaborate with healthcare teams for integrated care.
• Document progress using Home Care Home Base EMR.
• Educate patients and families on therapy practices.
**Qualifications:**
• California SLP license.
• BLS certification.
• 1•2 years in home health or hospice.
• Familiarity with Home Care Home Base.
**Contract Details:**
• 13•week contract, possible extension.
• 40 hours/week, day shifts.
• Overtime available.
• Flexible time off.
