We are seeking a Home Health Occupational Therapist for a 13•week contract in Long Beach, CA, with potential for extension. This role involves a 40•hour workweek during daytime shifts.
**Key Responsibilities:**
• Provide occupational therapy in patients' homes.
• Use Home Care Home Base for documentation.
• Conduct start of care visits and manage caseloads.
• Collaborate with a multidisciplinary team on care plans.
**Qualifications:**
• CA Occupational Therapist license.
• Basic Life Support (BLS) certification.
• 1•2 years of home health or hospice experience.
• Familiarity with Home Care Home Base EMR.
