Job Title: Rehabilitation PT • Day Shift
We're looking for a dedicated Rehabilitation PT with at least 1 year of skilled home health experience to join our home health team in Sonoma, CA.
Responsibilities:
• Deliver 1:1 patient care, up to 6 patients daily.
• Work a 5x8 hours day shift (08:00 • 16:30), including at least 1 weekend shift per contract.
• Maintain BCLS certification.
Requirements:
• Reliable transportation.
• Openness to occasional travel within a 50+ miles radius.
• Experience with Oasis preferred.
Additional Information:
• No standard scrub color required.
• Charting via EPIC.
• Free parking and mileage reimbursement at IRS standard rate.
• Local candidates and those with pending licenses are encouraged to apply.
This role does not require on•call shifts or unit floating. The work week is contractually guaranteed.
