**Job Title: Case Manager * LVN * Days**
Join our team in Woodland Hills, CA, as a Case Manager * LVN for a 13*week contract, working Monday to Friday, 8:00 am to 4:30 pm.
**Responsibilities:**
* Manage 20*24 Med Tele patients.
* Assess discharge readiness and communicate with RNs.
* Provide discharge education and resources.
* Identify and resolve discharge barriers.
* Facilitate patient flow through the hospital.
**Requirements:**
* Active CA LVN License.
* BLS Certification.
* 2+ years LVN experience in Case Management and Med Tele.
* Experience in discharge processes.
* Effective liaison skills between patients, families, and RNs.
* Travel experience preferred; local travelers welcome.
**Additional Information:**
* No on*call duties; floating as needed.
* COVID vaccine not required.
* Scrub color: Royal Blue or Navy Blue.
* PPE provided with on*site fit testing.
