**Job Description:**
We are seeking a Speech Language Therapist for home health services. Deliver quality speech therapy to patients in their homes, focusing on communication and swallowing improvements.
**Responsibilities:**
* Assess and create personalized treatment plans.
* Conduct therapy sessions for communication and swallowing.
* Collaborate with healthcare teams for integrated care.
* Document progress using Home Care Home Base EMR.
* Educate patients and families on therapy practices.
**Qualifications:**
* California SLP license.
* BLS certification.
* 1*2 years in home health or hospice.
* Familiarity with Home Care Home Base.
**Contract Details:**
* 13*week contract, possible extension.
* 40 hours/week, day shifts.
* Overtime available.
* Flexible time off.
