**Job Title: Home Health Physical Therapist Assistant**

**Location:** Concord, CA

**Role Overview:**
Provide essential physical therapy services to the geriatric population in their homes. Collaborate with healthcare professionals to ensure comprehensive care.

**Responsibilities:**
* Deliver quality therapy to home*based patients.
* Work with RNs and OT/PTs for integrated care.
* Manage a caseload of 6 patients daily.
* 13*week contract, Monday*Friday, 8:00 AM * 5:30 PM.

**Qualifications:**
* CA PTA license.
* BLS certification.
* 1 year of home health experience.
* Oasis/HCHB familiarity preferred; training available.

**Additional Info:**
* No on*call/holiday work.
* Flexible scrub color.
* Includes HR, HCHB training, and mentorship.
