We're seeking a dedicated Case Manager with a nursing background to join our team. This role is crucial to our local case management program.
Key Responsibilities:
• Implement and manage the case management program, including Care Facilitation, Utilization Management, Case Management, and Discharge Planning.
• Coordinate hospital•level care/utilization management systems and processes.
• Assist in departmental operations, discharge planning, and clinical quality improvement.
• Participate in financial and quality data collection, analysis, and reporting related to utilization management and performance improvement.
The ideal candidate will comply with all policies and procedures and possess the necessary requirements and specialty experience for the position. This is a day shift position.
