Home Health – Physical Therapist – Days

**Job Title: Home Health Physical Therapist • Days**

**Location:** Riverside, CA

**Contract Duration:** 8-13 weeks

**Schedule:** 40 hours/week, Day shifts

**Responsibilities:**
• Deliver physical therapy in home health settings
• Conduct assessments and create care plans
• Document using Home Care Home Base
• Perform start of care and follow-up visits

**Qualifications:**
• CA Physical Therapist license
• BLS certification
• 1•2 years home health/hospice experience
• Familiarity with Home Care Home Base
