Job description**Job Title: Home Health Physical Therapist**Join our team in Abilene, TX, for a 13•week contract, Monday to Friday, 40 hours weekly, no weekends or on•call.**Responsibilities:**• Deliver physical therapy in home health settings.• Perform SOC assessments with Oasis.• Create personalized care plans with patients and families.**Qualifications:**• Valid PT License and BLS certification.• 1+ year of home health experience.• Familiarity with HCHB and Oasis.**Additional Information:**• Potential for contract extension.• Flexible scrub colors.• Service area within 75 miles of Abilene, TX.Ideal for a committed Physical Therapist eager to impact home health care.
